An accurate, informative Network Directory allows our clients, case managers and claimants to easily contact your office to schedule an appointment or make a referral, and allows you to receive timely reimbursement for services.

Each year, we send demographic and affiliation information forms for your review. Please confirm or correct the information, and sign and fax the form to 1-973-776-2829. Doing so will help ensure that we have the most up-to-date information in our provider directory for your practice.

If your practice information changes at any point in the year, simply fax updated information on letterhead. The following changes can be submitted by fax:

  • Phone/fax number
  • Address, including new locations or the closure of existing locations
  • Physician(s) joining or leaving the practice
  • Tax Identification Number (TIN)
  • Practice name or ownership changes
  • Other changes affecting your practice
